Let's say I have FF account with American. I fly on a codeshare
flight which carries the number of the carrier X, but is operated
by Y. Now X and Y could have very different agreements with
American. So, whose rules apply on whether I get any miles,
or how many?

It depends:

1. I don't believe you would get miles on Y.
2. Whether you get miles on X depends on the rules set by that carrier
for AA miles.

Pick the carrier the code belongs to.
For example, JL..
For JL, you will see

"Codeshare flights marketed by Japan Airlines (flights with a JL flight
number) but operated by other airlines are eligible for mileage accrual."

Cathay, on the other hand, seems to restrict earning AA miles only for
flights on One World codeshares

Cathay seems to limit it to only codeshares on OneWorld carriers
"Cathay Pacific codeshare flights operated by any oneworld Alliance
member airline are eligible for mileage accrual when purchasing an
eligible fare. "

AFAIR the operating airline is what counts, and whether it is a
fully integrated partner or member of the relevant alliance.

The fact that your boarding card appears to be from a partner
airline, with that flight # is not relevant. But you can always
try to claim the miles with the boarding card/booking reciept.

I notice that if I buy a ticket online at aa.com, it has to be paid for
immediately, there is no "hold" of the reservation.

However, if I phone them, they can gold for 24 hours.

Does any body know if I can get a longer hold by going to "normal"
travel agents?

Sorry.. forgot to answer the original question.
Yes, going to a TA should get you time. However, since it can be done by
phone ($10 fee), or online ($0, plus miles), I don't see the need for
going to a TA.

In this case I want a special itinerary and flights that I have been
unable to duplicate at the aa.com site, although human agents
should be able to find it.

AA.com just spits out a few itineraries and expects you to
choose among them. I can see they are leaving out some of
the best possibilities, but I don't know how to enter the info!
I find the site to be good for generic round trips but frustrating
for anything complex like stops, one leg on a particular airline
or nonstop, etc. At least I don't know how to work it.

Where you doing a search by fare? Schedule?

Have you tried using the multi city function?

Multicity, by fare. The problem with schedule search is
I have no idea of the fares available on a flight. In this
respect I like ITA software, where you have some handle
on both schedules and fares.

The basic problem, in a sense, is mathematical. There are
so many possibilites that AA.com cannot possibly consider
or offer them all. Being able to specify parameters like number
of stops or "all airlines" for the entire trip is a very blunt
instrument.

So until you can specify a few things flight by flight (this
one nonstop, this one on BA or LH, etc) I see no alternative
to interacting with a person, on phone or face to face.

OK, I tried again. Here is where I think the problem was:

"Reservations containing one or more flights on airlines other than
American Airlines, American Eagle and AmericanConnection must be
purchased at time of booking..."

I guess we may still get some grace period on phone, and certainly with
travel agents. but not on aa.com if a non-AA segment is involved.

You also don't get a hold at aa.com if using a discount code.

I think it depends on who X and Y are.

For example, last October I flew from MCY to SYD on a ticket bought through
Qantas but it turned out that the flight was fully operated JetStar, Qantas
only had a code-share.

Qantas are in the same alliance as American, so I contacted AA who told me
that I could not collect AAdvantage miles from the flight since they don't
have any agreement with JetStar.
